I have that too. my problem was a mixture of things:
1.) my ball bearing exploded
2.) your fork sometimes pops out a tiny bit if the headset isn't tightened enough
3.) water and crap breached the seal

You could be having one those three problems just like pop your fork off clean your steer tube out re-grease it and you should be good to go I have that problem so much thats how I deal with it
